We are so excited to see how we can help you celebrate your special occasion. Please fill out our online Service Request Form. It should only take a few short minutes. The information from this form helps us determine how we can best assist you. Once it is accepted someone will contact you to schedule an appointment! Please note that it may take 7-10 business days to receive a response. If you are having an event emergency we ask that you please email us at info@fromthisdayforwardministry.org or call our boutique at 260-403-7323 in addition to submitting the application.

Corporate Event:

Are you planning a special event for work? Maybe trying to host a fundraiser on a budget? We have a brand new form just for Corporate type events! Click HERE to submit your request today! Due to high demand we do ask that you submit your request at least 30 days prior to your event. Applications submitted with less than 30 days notice may be subject to a rush fee. We know sometimes there are extenuating circumstances, so please indicate that on the form. Thank you for supporting our organization!
